Major Medical
Basic Plan (Blue Cross Blue
Options) PPO: Under this plan,
you have the freedom to use in network
or out of network medical providers. For
in network preventive care, the
deductible does not apply, your benefit
coverage is at 100% with a $15 per
office visit co-pay. If you use an out
of network provider, your benefit
coverage is at 50% after meeting the
annual out of network deductible. There
is also a $30.00 per office visit co-pay
for specialists, no referral is needed.
Prescriptions are $15.00 for generic
drugs, $30.00 for brand name and $50 for
non-formulary drugs. The employee pays
50% of the employee premium and 100% of
children and/or family.
Dental
Guardian: Preventive
care is paid at 100% with no deductible.
$5 per office visit co-pay. Other
services are subject to an annual $50.00
deductible ($100.00 per family) and
reimbursed at 80% except for major
services, which are reimbursed at 60%.
The annual maximum benefit is $2,500 per
participant.
Short-term Disability
Effective six (6) months after your date
of hire, you may enroll into the
short-term disability program. The
employee pays for this coverage. This
plan will pay a percentage of your
pre-disability earnings on a weekly
basis should you suffer an injury or
illness, which prevents you from
working. Short-term disability benefits
are considered taxable benefits.
Long-term Disability
Effective six (6) months after your date
of hire, you are automatically added to
our long-term disability policy. At no
cost to you, after 90 or more calendar
days of partial or full disability, this
benefit insurance will pay 66-2/3% of
your pre-disability earnings up to
$8,000 per month, less any benefits you
may be receiving such as state
disability, social security, etc.
Benefits may continue to age 65 and are
considered taxable income.
Aflac
We also offer many Aflac products, which
is a cost to you. Many of them include,
Cancer Care Insurance, Group Term Life,
Specified Health Event, Accident
Insurance and Short Term Disability.
401(k) Plan
Effective six (6) months after your date
of hire, you may enroll to save for your
retirement by contributing 3% of your
pre-tax earnings to the 401(k) plan. You
may contribute up to a maximum of
$13,000 per calendar year. BG&B will
match employee contributions at 25% up
to the first 5% of your contribution (up
to a maximum of 2.5% contribution rate)
immediately upon participation in the
plan. The company match is 100% vested
after 5 years of service.
Vacation and Sick time
Regular, full-time Employees are
eligible for vacation and sick time as
follows:
After your ninety (90) day
probation, the employee will accrue
one (1) day every two months.
After one (1) year of completed
service, the employee is entitled to
5 vacation days and six (6)
sick/personal days.
After two (2) years of completed
service, the employee is entitled to
10 vacation days and six (6)
sick/personal days.
After five (5) years of completed
service, the employee is entitled to
15 vacation days and six (6)
sick/personal days.
After ten (10) years of completed
service, the employee is entitled to
20 vacation days and six (6)
sick/personal days.
In the event that available vacation
is not used by the end of the
benefit year, employee will forfeit
the unused time.
Paid Holidays
The Company observes certain state
and national holidays. These
holidays are New Year's Day,
President’s Day, Memorial Day,
Independence Day, Labor Day,
Thanksgiving Day, and Christmas Day.
To be eligible for holiday pay,
employee must work the last
scheduled day immediately preceding
and the first scheduled day
immediately following the holiday.
BGB reserves the right not to close
during regularly scheduled holidays
based on company goals and
objectives. In these instances, all
employees are required to work as
otherwise scheduled.
